Step 1: New Client Intake Form
All new clients begin with a comprehensive intake. This step is required before your appointment can be confirmed.
Your intake allows me to review your skin history, current concerns, and patterns so your consultation and treatment can be approached intentionally, not generically.
Corrective care is a partnership.
Homecare is a required part of treatment and will be discussed and customized during your consultation to support progress between visits.
Please note:
- Appointments are not confirmed until the intake form is completed
- Incomplete intakes will result in automatic cancellation
- If your intake indicates that corrective care is not appropriate at this time, your appointment may be adjusted or refunded according to policy
At the end of the intake form, a required non-refundable booking fee is collected to reserve your appointment time. This fee secures your spot and is transferable one time if rescheduling is needed.
This process ensures your time and care are protected from the start.
